In-Home* and Telehealth and Assessment
We’re not just offering FREE in-home* and telehealth assessments for members – we’re paying $50 for members to complete them! We’re working with Advantmed to offer this benefit and support your health. Call our Member Services team at (855) 447-2900 for details.
You will receive a call from Advantmed (406-559-2066), or call 1-800-367-5319 to set up your appointment. Fill out the form below if you would like to participate in the free in-home* or telehealth visit. As soon as we receive your information, someone from Advantmed will reach out to you to schedule your visit within 72 hours!

Home Telehealth Assessment - Checklist
You will not receive an invoice or bill for this screening. The CO-OP will pay for the assessment and any in-home testing that is performed. It’s completely free! The Nurse Practitioner will share all of the information and test results with your doctor.
The health assessment does not replace your doctor's visit. This is an additional benefit that allows you to take the time to discuss issues that matter to you. The Nurse Practitioner will not change or add medications or change the treatment plan your doctor has given you.
Your 50.00 check for completing the health assessment will be mailed you. You should receive the check no later than 90 days after your appointment.
