Centers of Excellence Program
As a member owned health insurance CO-OP we not only value your overall health but understand that costs associated
with procedures and services can cause hardships both financially and logistically. As such, we have created a program
called “Centers of Excellence”, “COE”. This program was designed with you in mind and geared towards reducing your
out-of-pocket costs while ensuring access to quality of care.
At a Glance:
Member deductible will be waived for services provided at a COE**
To qualify, you must receive a prior-authorization. This can be obtained by calling 801-587-2851.
Travel expenses related to the member will only be reimbursed for care done at MHC’s approved Centers of
Excellence (COE) and must be out of state or more than 120 miles from the patient’s residence
All travel related reimbursement requires prior-authorization, which can be obtained by calling 801-587-2851
Travel expense reimbursement is limited to $10,000 per calendar year per member
Air transportation will be arranged by and billed to MHC
Transportation covers the member and one caregiver
Transportation by personal vehicle will be reimbursed at the IRS standard mileage rate, round trip, from your
home zip code to the zip code of the provider location
Lodging will be arranged by and billed to MHC
Member and caregiver will be given a daily per diem equal to the current federal meals and incidental daily rate
for the location in which they receive treatment

Per-diem doesn’t apply to member when inpatient
**Due to federal tax laws, an exception to the waived deductible occurs when a member is enrolled in a qualified high
deductible health plan; however, your out-of-pocket costs might still be reduced by having services performed at our
COE**
